Indiana has filed for an extension of an 1115 waiver for its Healthy Indiana Plan (HIP) 2.0, with changes that include both a work requirement and moving premium payments from a fee based on income to a flat fee -- two waiver provisions that CMS has never previously approved for a state Medicaid program.
