CMS Announces Participants In Medicare 'Million Hearts' Demo

CMS announced 516 awardees in 47 states will participate in the Million Hearts Cardiovascular Disease Risk Reduction Model. The demonstration is designed to decrease cardiovascular disease by analyzing a patient's risk for heart attack or stroke and using prevention interventions. Around 20,000 health care practitioners and over 3.3 million Medicare fee-for-service beneficiaries will participate in the five-year model, CMS said.
