Safety-net hospitals are upset that CMS' final Medicaid managed care rule codifies an often-ignored ban on states paying extra for services that capitated rates don't adequately reimburse, but they are relieved that states get a decade to figure out how to cover those safety-net hospital costs in other ways, as doctors and nursing homes must comply in five years.
