CMS proposed Tuesday (May 26) an overhaul of the rules for Medicaid managed care plans that applies a national medical loss ratio to Medicaid managed care plans, adds long-term services, creates a grading system similar to Medicare Advantage star ratings, establishes network adequacy standards and trues up the rules for managed care among Medicaid, Medicare and private exchange plans.
